Direction Home’s network of 12 Ohio Area Agencies on Aging are experts in addressing social determinants of health in older adult populations. We have been promoting independent living for older adults and people with disabilities for the past fifty years.
We are a statewide network that works with a number of different types of payors to deliver services to members in every single county in Ohio.
We manage over 4100 contracts with local community-based organizations to deliver direct services, such as meals, transportation, personal care, and home modifications in all regions of the state.
We can facilitate the delivery of targeted home and community-based services to address the needs of high-risk older adults – with a focus on diverting members from institutional long-term care facilities, reducing utilization of acute inpatient services, and stabilizing at-risk members in the community, with additional home and community-based services.
We provide a single source of referral for members that require a range of home and community-based services, person-centered planning, and local care coordination and evaluation of supplemental benefits.
